The gap between how procedures are written and how work actually gets done.

Technical work instructions are carefully prepared. They're complete, they're correct, and they're in the system. But on the shop floor, they're rarely consulted, not because people deliberately ignore them, but because they're not accessible at the moment they're needed.

Looking up a PDF via a laptop in the site office, printing it, taking it along, then trying to figure out what step 14 means while working in an awkward position: that's the reality. People skip steps, do it from memory, or ask a colleague who may not know exactly either.

That's not a discipline problem. It's a design problem. We build work instructions designed for the situation in which they're used, on a tablet or phone, with visual steps, checkboxes and warnings exactly where they're relevant.
